Lowville Youth Lacrosse’s mission is to provide a safe, community-minded and income-sensitive lacrosse program. With this in mind our non-profit organization allows players to borrow gently-used equipment from our organization’s inventory for a variety of different reasons including: (1) trial participation for beginners; and/or, (2) financial hardship. We have determined that your child/family meets one of the aforementioned qualifiers. As such, we are offering you a free equipment rental for the term of 6 months. You must return your equipment during the scheduled drop off times. Failure to do so will result in a $100 fee and the inability to rent from us again.
I, Parent/guardian of minors full name agree to treat the equipment I have borrowed from Lowville Youth Lacrosse with great respect. If the item(s) that I have borrowed is/are damaged beyond repair, lost or stolen. I agree to replace the item(s) with an item of equal or greater quality/condition. The item(s) that I agree to borrow is/are listed below.
